Shared Space Agreement

Room Rate & Billing

Hourly Rate: Usage is billed at a rate of $15.00 per hour, and will be charged at the time of booking.

Inclusions: This hourly rate is all-inclusive of utilities, Wi-Fi, and facility maintenance.

Security Deposit: No security deposit is required at this time.

Scheduling & Shared Access

Booking your time: Room reservations will be handled via our website: simply-wellness.com/scheduling Lessee is responsible for ensuring their bookings are accurately reflected in this system. All room reservations must be made a minimum of two hours in advance. Changes or cancellations must be submitted at least 48 hours prior.

Non-Exclusive Use: Lessee acknowledges that the treatment room is a shared space used by other tenants and Lessor’s employees on other days/times.

Booking: Use of the room must be scheduled in advance via provider.simply-wellness.com.

Turnover: Lessee must vacate the room promptly at the end of their scheduled block. The room must be left in "ready-to-use" condition for the next provider (see Addendum A). 

 

Access, Security & Storage

Access: Lessee will be provided with a unique entry code. Lessee shall not share their unique entry code with any third parties.

Hours: Access is permitted during scheduled appointment sessions.

Storage: Lessee will be provided one (1) dedicated shelf in the storage room. All supplies must be kept within this footprint.

Liability: Lessor is not responsible for any lost, stolen, or damaged equipment or supplies stored on-site. 

Professional Requirements & Branding

Licensing: Lessee must provide proof of current State Board Certifications and a valid Business License prior to the start date and annually thereafter.

Concierge Services: Lessee is responsible for their own patient intake, greeting, and checkout. Lessor’s front desk staff will not provide administrative support for Lessee’s business.

Branding: No external signage, window decals, or door branding for Lessee’s business is permitted.

Insurance & Liability

Coverage: Lessee shall maintain Professional Liability and General Liability insurance with limits of no less than $1,000,000 per occurrence / $3,000,000 aggregate.

Proof of Insurance: A Certificate of Insurance (COI) naming Simply Wellness Concierge: a Physician Assistant Corporation as an additional insured must be provided prior to use of the space.

Indemnification: Lessee agrees to indemnify and hold Lessor harmless from any claims, damages, or legal fees arising out of Lessee’s professional services or conduct.
